How Asian Noses Differ From Caucasian Noses
Asian rhinoplasty does not try to create a Caucasian nose but rather to alter the nose so that it flatters the Asian face. Asian noses tend to have a flatter nasal bridge and a weaker nasal tip. The Asian nasal bridge starts a little lower around the eyelash level. The cartilage of the tip may be weak, requiring reinforcement to adjust the shape and projection of the tip. If the tip is round, it is not due to excess cartilage (as with Caucasian noses) but due to the weakness of the cartilage. Thus, Asian rhinoplasty must focus on adjusting these areas so that the nose appears more balanced with the other facial features.
Asian Rhinoplasty
While Caucasian rhinoplasty often attempts to minimize the nose, Asian rhinoplasty often makes the nose larger and more prominent. The focus is often to build up the nasal bridge and reinforce the cartilage of the tip. Southern Asians may also need slight narrowing of the nostrils, though this may be achieved with tip projection alone.
Dr. Lee primarily uses natural tissue and cartilage to build up the bridge of the nose. In some cases, rib grafts may be used to retrieve enough cartilage for the nasal bridge. If necessary, Dr. Lee may use a silicone implant, but he prefers to use cartilage because it is soft and pliable. Natural tissues are also preferable because there is a lower likelihood of implant rejection.
Dr. Lee may also use cartilage from the nasal septum or the ear to define the nasal tip. A rigid piece of cartilage can reinforce and project the nasal tip upward so that it looks more attractive. If cartilage grafts are taken from the ear, the ear usually returns to its normal shape in only a few days. Northeastern Asians are typically able to use septal cartilage, while Southern Asians may need to use ear cartilage due to insufficient septal cartilage for adequate tip support.
When patients request or require a dramatic change to their nose, Dr. Lee may use rib cartilage. Ideally, Dr. Lee obtains a patient’s rib using a one to two-inch incision below the breast fold. This adds 30 minutes to the procedure. In patients who are not suitable candidates, donated cartilage from a tissue bank is an alternative.
